Considerations and Preparation:
To be considered for eligibility for a Distribution Payment from the Summit Planning Group Fund, you must provide the informationrequested in this online Certification Form and submit a release throughthe Fund Administrator’s online claim system by March 31, 2026. Failure to do so may result in your claim being rejected and you being precluded from any recovery from the Summit Planning Group Fund.
Timely submission of your Certification Form does not guarantee you will be eligible for a Distribution Payment from the Summit PlanningGroup Fund or that you will be compensated for your claimed loss.
